I guess this is a custom format rather than standard modfile.

Yes, it had to be a custom format: Instead of using 4 channels that are mixed together on the three YM channels, he instead decided to go for only three channels of music, each mapped directly to one of the volume register of the YM.

Based on my own tests at doing real time mixing on the 6502, his approach worked much better, and you even have (a bit of) free cpu time!

Then, it definitely has to be 3 channels. No way to mix the channels in real time.

It sounds awesome, knowing what's the hardware behind the scene... Is it "just" trying to approximate a sound sample with the 3 channels of the YM chip ? Or is there some real-time mixing going on, like for a MOD player ? For the first case, Wikipedia page about YM chip mention MSX guys who found a way to precompute the sequence of instructions for the YM, so that it best approximate a given sound sample.
